INTRODUCING SELF-PAY JACKPOT

Self-Pay Jackpot™ is a new and enhanced way to play where you can transfer your slot jackpot directly to your favorite slot machine.

Enrollment is FREE. To use Self-Pay Jackpot™, you must have a Wild Card.

You may apply to participate in the Self-Pay Jackpot™ Program at the Casino Credit Office, Casino Cashier or with the slot customer service representative on the casino floor.

You may apply to participate in the Self-Pay Jackpot™ Program by completing and signing the Self-Pay Jackpot™ Application. You must present your Wild Card and two (2) forms of government-issued photo ID (at least one of which has a photo).  Upon acceptance of your application, the Casino will retain your electronic signature for use in completing Forms W-2G. If you also participate in the Power Bank ™ Program and play with funds drawn on a Casino Credit Account, your electronic signature may also be used in preparing markers.

Self-Pay Jackpot™ is simple, safe, secure and convenient. Self-Pay Jackpot™ gives you the power to transfer your slot jackpot directly to your slot machine.

Simply insert your Wild Card into your favorite slot machine and input your PIN. The digital screen on the slot machine will provide instructions for downloading your jackpot. The amount of your jackpot will appear as a credit in the credit meter on the slot machine.

Your jackpot cannot be accessed without your PIN. You must keep your PIN confidential and not share it with anyone.

If you also participate in the Power Bank™ Program and are playing with funds from your Casino Credit Account, the credit balance remaining on the credit meter of the slot machine at the end of your session (which occurs when you remove your Wild Card) will be deposited back into your Casino Credit Account to repay the amount of credit you used during that session.  If the credit balance on the credit meter of the slot machine is not enough to repay all of the credit you used during the session, the entire balance will be applied to pay down the balance of your Casino Credit Account and the Marker Bank Cashier will prepare a new marker for the outstanding balance of your Credit Account using your electronic signature. Any remaining winnings will be printed on a ticket at the slot machine which you may cash out at the Casino Cashier or use to play another slot machine. Any winnings of $1,200 or more from a single wager will be reported on a Form W-2G that will be filed with the IRS.

Yes. You may terminate your participation at any time by following a few simple procedures at the Casino Credit Office.

• Must be 21 years or older.
• Must be a U.S. citizen or resident.
• Must have a valid Wild Card.
• Must sign an application and agree to the terms and conditions described therein.
• Must provide two (2) forms of government-issued ID (at least one with a photo).
